Bump ForkRevision to 5 and promote the unreleased CHANGELOG section (#149 password column widening and pre-enable check, #151, #153) to 10.0.0.5. Also fix the SQL Server pre-check query in the password migration doc: filter by TABLE_SCHEMA = SCHEMA_NAME() to match what the inspector actually queries, and select DATA_TYPE, COLLATION_NAME and the database default collation so readers can decide whether the ALTER in section 3 needs COLLATE. Sections 2, 3 and 5(a) were validated on SQL Server 2019 (compat level 150, database collation differing from the server default). Refs #156 Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
